Each plan is designed for a different level of need, from gentle weekly check-ins to more frequent support or short-term help after a hospital or rehab stay.
Family Peace of Mind
The Family Peace of Mind plan is best for families who want consistent weekly reassurance and another trusted set of eyes in the home.
This plan includes 1 scheduled wellness support visit per week.
Typical support may include:
- General wellness and comfort check-in
- Medication reminders only
- Routine and appointment reminders
- Hydration and nutrition awareness
- Comfort-focused conversation
- General observations of routine or changes
- Family peace-of-mind update after the visit
Best for:

This plan includes 2 scheduled wellness support visits per week.

- Additional family updates during the week
- More support with routine and appointment organization

Post-Hospital & Post-Rehab Organization Support
Smooth Return Home is a short-term support package designed for families navigating the first two weeks after a hospital stay, rehab stay, illness, fall, or major change in routine.
This package includes 3 visits over 2 weeks:
- Visit 1: 90-minute initial return-home visit
- Visit 2: 60-minute follow-up organization visit
- Visit 3: 60-minute peace-of-mind check-in visit
Typical support may include:
- Discharge paperwork organization
- Follow-up appointment reminders
- Medication reminder discussion
- Family question list for the physician or pharmacy
- Comfort and routine review
- Family updates after each visit
- Final next-step recommendation
- Discussion of whether ongoing wellness support may be appropriate

Families who feel overwhelmed after a hospital or rehab discharge and want help getting organized during the transition home.
